You do not need a referral to see a Physiotherapist.

If the treating Physiotherapist thinks it is necessary they will refer you onto the appropriate Health Care Professional eg, GP/Specialist or Radiology

WHAT TO BRING

If referred by a Medical Practitioner

Bring the referral letter and X-rays/ Scans if available

Worker’s Compensation/Third Party Claims

Bring your Workers’ Compensation Medical Certificate or any insurance information/letters this will include your claim number and case manager.

Private Health Insurance Holders

Bring your Private Health Insurance card this will help with payment as we have HICAPS and you can claim your rebate immediately and just pay the gap fee.

DVA

Bring your Gold Card and D904 DVA request referral form to your first visit this is from your GP or Specialist

CLOTHING:

You will need to expose as much of the injured area as possible. At the clinic we have shorts for you to change into but if you would feel more comfortable in your own shorts or t-shirt please bring them with you to each physio appointment.
